Vocational Rehabilitation Services Application Instructions
Please read the "Consumer Information Handbook" (in Rich Text Format) for an explanation of Civil Rights, Eligibility Requirements, Confidentiality, Appeals Procedure, and the Client Assistance Program before proceeding. You may also choose to listen to the "Consumer Information Handbook". After reading, or listening to, the "Handbook" if you feel that you qualify as a DOR consumer and you believe that the Department can assist you in achieving your employment goals, then do one of the following:
Printer Friendly: Use one of the printer friendly forms,
complete it, then locate the nearest Department of Rehabilitation office and
Mail the application to the office and they will contact you for an appointment with a rehabilitation counselor,
Phone the office for an appointment with a rehabilitation counselor and bring the application with you to your appointment, or
Bring the application to the office and they will schedule an appointment with a rehabilitation counselor for you.
If you need assistance completing the application, the rehabilitation counselor can assist you.
